• Conduct research with human participants. Contact your U of T Research Ethics Board (REB).
• Handle live animals/carcasses/tissue or have access to a vivarium. Contact the Animal Research Oversight & Compliance Office.

Lasers

I work with class 3B or class 4 open beam lasers

I have never taken any laser safety training
I have taken full laser safety training at UofT
I have only taken laser safety theory training at UofT
I have taken laser safety training in other institutions

I work with an enclosed laser system
I work in a laser lab but I will not operate the laser
I work with a fiber-coupled laser
I’m a Laser Permit Holder, but I don’t operate lasers
