Online Accident/Incident eForm

Getting Started

This form should be completed and submitted within 24 hours of an incident or injury involving a UofT employee, UofT student, non-UofT contractor, or visitor (including visiting students conducting research). This form may also be used to report incidents like near misses, where there was no injury, but the potential for injury existed.

For injuries or incidents involving employees, the employee’s supervisor/manager must complete the form. For injuries or incidents involving students, non-UofT contractors, or visitors, the form should be completed by a University of Toronto employee.

If you require medical care, please go to a health care provider. Occupational Health and Safety does not provide primary care, but only occupation-related consulting services.

Important Notes About eForm

  1. Before you start the form, obtain as much details of the incident and personal information of the employee/student/contractor/visitor involved as possible. If you do not have all the details before starting the form (e.g., phone number), enter placeholder number/text to complete the submission of the report (e.g., 111-111-1111) and note this in the free text section. Missing information can be e-mailed to ehs.office@utoronto.ca once available.
  2. Once the form is submitted, a copy of the report will be sent to the e-mail addresses provided on the form. If you do not receive a copy of the form, contact the EHS office.
  3. The eForm requires UTORid authentication (UTORid and password).

Note: Planned monthly maintenance is scheduled on the last Wednesday of each month from 10 AM to 1 PM, during which this form will be unavailable. Please save your progress and submit the form outside of this maintenance window to avoid potential data loss.  

For emergencies or critical injuries, contact ehs.office@utoronto.ca during the maintenance period. Please note if you e-mail us, EHS will follow up and ask you to complete an online form once it is available again. 


Additional Guidance for Staff and Students

  1. Failure to report injuries to employees which result in health care or lost time from work (other than the day of injury) may result in a financial penalty to the applicable department.
  2. Students who are injured on UofT property and/or in the course of a UofT-sanctioned activity (e.g., research, academic or other UofT programming, such as fitness classes, student clubs, etc.) should immediately report the incident to their UofT contact (e.g., academic contact or athletic front desk staff). The UofT contact will then complete and submit the Accident/Incident eForm.

  3. Students who are injured while employed by UofT (e.g., TA for class) should immediately report their incident to their workplace supervisor who will then submit the Accident/Incident eForm. If the student is unsure of their employment status at the time of their injury, they should contact their divisional HR office.

  4. Students who are injured while they are on a placement should immediately report the incident to their placement employer/supervisor and to the UofT Placement Coordinator (placements@utoronto.ca) and complete the Student on Unpaid Work Placements Accident Report.
